Take OH-3 N for 6 miles, turn right onto Center Rd. We're 0.4 miles up on the left.

Do I need to schedule an appointment?
You can walk in for primary care just like urgent care. That said, scheduling ahead guarantees your preferred provider and time slot — call us to set it up.
Will I see the same provider each time?
That’s the goal. When you choose a primary care provider at one of our clinics, we’ll do our best to schedule you with them consistently so they know your history.
Is primary care covered by my insurance?

Can I transfer my records from my current doctor?
Absolutely. We’ll help you request records from your previous provider so your new clinician has your full medical history from day one.
