Vaccination records

Bring everything you have. We can fill in the gaps but can't recreate missing history.

Form I-693 (if downloaded)

Partially completed if you've started it. We'll finish, sign, and seal it.

List of medications & conditions

Any current prescriptions, allergies, or chronic conditions for the medical history portion.

Common questionsn

How long does the exam take?
Most patients are finished within 45 to 60 minutes. If you need additional vaccines or lab work, expect closer to 90 minutes.
In most cases, the same day. If lab results need to come back first (such as TB blood tests), we’ll have your sealed form ready within a few business days.
No. USCIS requires the envelope to be submitted sealed and unopened. If it’s been opened, you’ll need to return for a new sealed copy.
Per current USCIS guidance, Form I-693 is generally valid for 2 years from the date the civil surgeon signs it, as long as you submit your application within that window. Always check the latest USCIS rules before filing.
