Take home a sealed envelope ready to file with your USCIS application.
What to bring

Government-issued photo ID
Passport, driver's license, or state ID.
Vaccination records
Bring everything you have. We can fill in the gaps but can't recreate missing history.
Form I-693 (if downloaded)
Partially completed if you've started it. We'll finish, sign, and seal it.
List of medications & conditions
Any current prescriptions, allergies, or chronic conditions for the medical history portion.
